许太史，字敬之，许昌人。其祖琰，王为曾祖，其父肅，是以姓許。汉末避难于豫章南昌，因家焉。赤乌二年己未，其母夫人梦金凤衔珠落掌中，即生真君。

真君自幼聪慧，姿容秀伟，与物无忤。一日狩猎射鹿，不意子落于地，而鹿母犹顾舐之，为感悟弃弓矢，以学道。博览经史、天文地理、音律五行讖緯神僊术尤精。

闻西安吴猛得至人丁义神方，便往师之悉传其秘。与郭璞访名山求善地，为栖真所居。在逍遥山金氏宅定居玉隆万寿宫中，每日事事为心，有丹井药臼存焉，不求闻达。

乡党化其孝友交游服其德义，有卖铁灯笼者夜然燈见漆剝处乃金也，翼日访主还之。不取非义物郡举孝廉不就朝廷屡加礼命不得已始以晋武帝太康元年起蜀郡旌阳令时年四十二岁。

视事初誡吏胥去贪鄙除煩细脱囚縶悉开谕以道吏民悦服愿自新发擿如神吏不敢欺听訟必先教以忠孝慈仁忍慎勤儉近賢远姧去貪戢暴具载文誡言甚详悉复患百里远难户晓乃择秀民有德望与耆老可语者委募能率故争竞风日销久而至无訟。

先是岁饑民无输租郡邑繩法率民多流移真君乃灵丹点瓦礫使潜瘞县圃一日籍未输纳者咸造庭使服力后圃民钁地获金得输纳遂安堵邻境流民慕德惠来依附众多至户口增衍属岁大疫死十七八真君用授神方拯治符咒所及登时愈至沉痾疾无不痊传闻他郡病相继千计於是标竹江外十里置符水其中俾就竹下饮皆瘥病不能自至汲归饮亦获痊安蜀民謠曰：人無盜竊吏無奸欺我君活人病無能為後江左來汲水旌陽真君乃咒水器置符其中令持歸江濱植竹標所俾病飲江左亦良愈今號錦水屬瑞州高安縣
